This randomized controlled trial evaluated the effects of testosterone therapy in 790 men aged ≥65 years with low serum testosterone (<275 ng/dL) and symptoms of hypogonadism. Over one year, treatment with testosterone gel successfully raised circulating testosterone to mid-normal levels seen in younger men. The most consistent benefit was observed in sexual health, with significant improvements in sexual activity, desire, and erectile function. There were also modest improvements in mood and depressive symptoms, suggesting some neuropsychological benefit. Safety outcomes were comparable to placebo, though the study was not powered to assess long-term risks. Overall, the findings indicate that testosterone replacement in older men provides targeted benefits (primarily sexual function) rather than a comprehensive enhancement of energy, physical function, or overall vitality.
